• Elder Law Planning Checklist

  • Generally, it helps if you can bring copies of the following documents and information to our initial Elder Law planning meeting:

    • Names, addresses, phone numbers, and social security numbers of your children
    • Deed to your house and most recent County Tax Receipt
    • Most recent statements for all bank accounts and investment accounts
    • List of face values and issue dates of all savings bonds
    • List of face values, cash surrender values and death benefit values of all life insurance policies (you may need to request this information from the life insurance company)
    • List of face value and death benefit values of all annuities (you may need to obtain this information from the company managing your annuity)
    • List of all stocks held, including the number of shares and issue dates
    • Burial Plot Deed and documentation of any pre-arranged funeral and burial
    • Annual statements of social security, pension, and other income benefits
    • Any prior gift tax returns filed
    • List and details of any outstanding debts
    • Your current Will, Power of Attorney, and Health Care Proxy, if any
